premium member should have 3slots says I have them all used but only Kiillz and Pennx are there should have one more

Slots are not cleared even if you delete all the characters from a given game account, you have to either reset them or ask me to reset them, that is the only way.
I have reset your game slots.

No worry, it's located under "My account", you have a link named "Free your game slots" in the Premium Subscription section.
