This screenshot shows a drop from the warbone spearman in lava, but when you click on the mob name to get specifics, this drop is NOT in the list? http://norrath.no-ip.org/warbone.bmp

This item: Serpentskin Eyepatch also shows on the incorrect mob. Similar problem? Interesting both are drops off very rare mobs in dagnor's cauldron.

I need to look into it but it shouldn't come from TLP random servers as I treat them differently and they have their own loot tables.
